What Trillium Meds actually is

Trillium Meds is a telehealth distributor brand — not a compounding pharmacy. The brand's role is intake, prescriber routing, and shipping logistics; the actual compounding and dispensing happen at a separate state-licensed pharmacy that Trillium routes the prescription to. The distinction matters because the buyer signs up with Trillium, pays Trillium, and reviews Trillium on Trustpilot, but the vial they inject was compounded somewhere else by someone else under a different state license and a different FDA inspection history.

That structure is the same model used by Big Easy Weight Loss, Rift, Brello, and Fifty410. It is the dominant 503A-tirzepatide telehealth shape in May 2026. The audit work for any pharmacy in this category is mostly the audit work on its partner roster.

Why Trillium ranks well in buyer surveys

The May 2026 monthly survey on r/tirzepatidecompound — the largest buyer-side community for compounded GLP-1s — recorded a 93% recommend rate for Trillium among respondents who had used the service. That was the highest score in the cohort the survey covered, ahead of Rift, Big Easy Weight Loss, Fifty410, and Brello. Trustpilot reads similarly: 4.5/5 "Excellent" across a broad review base. Recurring themes in both sources are fast shipping, accessible customer support, and a sense of named-owner ownership on the brand side that the competing distributors do not surface as well.

That signal is real and we don't discount it. The question this audit asks is what the signal doesn't cover, which is the regulatory record of the pharmacies that fill the prescriptions Trillium routes.

Who actually fills your prescription

Trillium publishes its partner-pharmacy roster on its own website — a transparency point in its favor relative to Rift, Brello, and Fifty410, which either redact the list or expose only one partner. The named partners are Hallandale Pharmacy (Pharmcore Inc.), Seven Cells Pharmacy (Purformance Wellness Pharmacy LLC), OptioRx, Miller's Pharmacy, ProRx / PerfectRx, and BPI Labs / Vita.

What Trillium does not publish is the per-fill assignment. A buyer who completes intake today does not know — before paying — which of those pharmacies will compound and ship their vial. That gap is the single biggest piece of disclosure friction in this audit. In every other regulated retail-pharma flow the consumer can see the dispensing NPI on the label before purchase; here, that information surfaces only after a vial arrives. We've flagged it as the highest-priority item to press on if this audit upgrades to a first-hand tier.

The Hallandale FDA record

The single biggest risk in the Trillium partner stack is Hallandale Pharmacy, the Fort Lauderdale 503A operating as Pharmcore Inc. dba Hallandale Pharmacy. The FDA's public inspection record for Hallandale is the densest of any pharmacy in the v0 cohort:

  • 2018 — Form 483 observations against Hallandale's sterile compounding operation.
  • March 11, 2020 — FDA warning letter to Pharmcore Inc. dba Hallandale Pharmacy citing insanitary conditions in sterile compounding. Warning letters are the most serious pre-enforcement signal the FDA issues short of consent decrees or seizures. The full text is linked in the citation block.
  • 2021 — Form 483 observations during follow-up reinspection.
  • May 3, 2022 — warning-letter closeout, indicating the agency evaluated the 2020 concerns as resolved at that snapshot.
  • June 13, 2025 — Form 483 observations issued during the most recent inspection cycle. This 2025 action is the one that converts Hallandale's record from "historical" risk to "current" risk for any Trillium buyer who fills today.

Secondary partner Seven Cells (Purformance Wellness Pharmacy LLC, in Stuart, FL) received a 483 on December 9, 2022. The remaining named partners — OptioRx, Miller's, ProRx / PerfectRx, BPI Labs / Vita — did not surface FDA actions in the public-record pass we ran for this audit; absence of evidence is not evidence of absence, and a first-hand audit would cross-check each pharmacy against the FDA inspection database and PCAB directory directly.

What buyers consistently report

Outside the regulatory question, buyer feedback is consistent across sources. Trustpilot's 4.5/5 base, the May 2026 Reddit survey's 93% recommend rate, and individual comment threads in r/tirzepatidecompound and r/Semaglutide all describe a service that ships fast (typically 2–4 day delivery), supports cleanly, and does not surface the cancellation-friction grievances that drag down Brello. The brand-side operator is named and accessible in community channels, which is rare enough in this category to be worth noting.

The structural caveat: buyer satisfaction with a distributor measures the distributor's UX, not the compounder's sterility practice. The Reddit survey was not designed to ask "which partner pharmacy filled your last vial" and the Trustpilot reviews don't either. So the high satisfaction reading and the partner-FDA record are not in conflict — they're measuring different things.

What's missing from public records

This audit is the public-data tier. To upgrade Trillium to a full audit tier we'd need:

  • A first-hand consult and fill placed by the vialaudit desk, with the per-fill partner-pharmacy disclosed pre-purchase if available, or documented as undisclosed if not.
  • The named-prescriber identity and NPI number, with state-medical-board cross-checks for the states Trillium ships to.
  • Per-state non-resident pharmacy license verification for each named partner in the Trillium roster, pulled directly from each state's Board of Pharmacy database.
  • The Trillium written refund and cancellation policy as published, archived against any future quiet revisions.
  • A snapshot of any pending or recent state-board enforcement actions against the partner pharmacies (the Hallandale record indicates a 2022 state referral was made; we did not independently verify the outcome).

We have not done that work yet. This profile reflects what is verifiable from public records as of the last-audited date.

Editorial note — the buyer-survey winner is not the audit winner

This profile is the editorial spine of the v0 compounding-pharmacy vertical: the popularity leader is not the audit leader. Trillium has the strongest buyer-experience signal in the cohort. It also has the highest-risk partner-pharmacy FDA record. Both can be true; the audit is designed to surface that divergence rather than confirm the popular vote.

A buyer optimizing for shipping speed, customer support, and recognized brand recall on r/tirzepatidecompound has reasons to choose Trillium that this audit does not contradict. A buyer optimizing for the lowest regulatory risk on the actual vial — the question this rubric is designed to weight — should look at Fifty410's thinner FDA paper trail on its named partners before committing to Trillium's Hallandale-heavy fill flow.
